This position requires direct care of individuals, along with providing program support services in the home as well as community settings. This individual is responsible for completing monthly and daily program documentation. This individual participates in meetings and advocacy groups pertaining to the betterment of our individuals and program at large. Prior knowledge of and/or education regarding working with individuals who have Developmental Disabilities is essential.

DUTIES AND RESPONSIBILITIES

• Assist individual’s with ADL’s
• Provides individual support and encouragement
• Advocate for individual rights
• Attend CSP meetings and other meetings as deemed necessary by management
• Strong verbal and written communication, Demonstrate time management
• Facilitate in service trainings
• Demonstrate time management
• Transportation
• Administer medication as needed
• Teach compensatory strategies within the home and community setting
• Communicates effectively with individuals, staff, and community affiliations
• Maintains documentation in compliance with all licensing agencies
• Performs crisis intervention as needed
• CPR and First Aide certified
• Transport individuals as needed
• Medication Management
• MANDT-Training re: behavior management
• Other duties as assigned

MINIMUM QUALIFICATION REQUIRED

• Must have high school diploma or GED
• Must have a valid Virginia’s driver’s license
• Child Protective Services Check
• Successful completion of a Comprehensive Criminal Background and Central Registry Check

Avail Health Services offers a wide range of services provided by our skilled and compassionate caregivers. Depending on the needs of our clients, we are positioned to offer care for just a few hours a week or, if needed, round-the-clock, in-home care. We’ll work to customize an individualized personal care plan that fits the specific needs of our clients. This position provides direct care services to clients in their homes. Our caregiver will provide assistive services such as but not limited to meal preparation, transportation, medication management, daily hygiene assistance and other personal care activities.

Key Duties and Responsibilities:

• Follows the instructions of the Personal Care Aide Supervisor and Nursing Director in providing personal assistance services.
• Provide assistance with hygiene such as bathing, oral care, and dressing
• Assist with food preparation; serve food, performs errands, light housekeeping, laundry, wash dishes
• Have knowledge of AHS policy and procedures
• Perform and record accurate measurements (i.e. vital signs, intake/output as defined care plans).
• Observe and report any safety hazards found in the client’s home or any significant observations regarding the client.
• Attend staff meetings, attend/complete annual in-services, and participates in orientation of new employees.
• Report client complaints to the Personal Care Aide Supervisor or Nursing Director
• Be trained in first aid and CPR.
• Maintain client confidentiality/adheres to HIPAA requirements and agency policy/procedures manual.
• Possess the ability to follow written and oral instructions.
• Report abnormal findings in client’s conditions as observed and per service plan to Personal Care Aide Supervisor or Nursing Director.

Minimum Knowledge and Experience:

• Must have a High School Diploma or GED
• Be trained in first aid and CPR
• Must have a valid Virginia’s driver’s license
• Proof of automobile insurance
• Successful completion of a Comprehensive Criminal Background and Central Registry Check
